A STRENGTH BASED CASE MANAGEMENT PROGRAM OFFERING ACCESS TO PRIMARY MEDICAL CARE AND SUPPORT SERVICES
The P.O.W.E.R. Program is a project designed to focus on your personal strengths and beliefs in achieving the maximum benefits available to individuals who are newly diagnosed with HIV infection or for those who have not received medical care for at least six months.
P.O.W.E.R. embraces and accepts individuals regardless of their perceived or real impression of health care service delivery systems and approaches the whole individual by working in close collaboration with case managers, community health outreach staff, as well as a broad menu of service providers.
P.O.W.E.R. will link you with services vital in improving the quality of life for individuals and families. In addition to immediate access to medical care and life-saving medications, P.O.W.E.R. case managers will customize a plan with your complete involvement and direction and can also assist you with:
þ Substance abuse and mental health counseling and treatment
þ Support Groups
þ Child Care
þ Patient Education
þ Nutritional assessment and intervention
þ Nursing Care
þ Dental Care
þ Food Pantry
þ Employment preparedness
þ Referral to Housing Programs
þ Referral to Spiritual counseling
þ Risk reduction education
þ Family Counseling
þ Emergency Financial
In addition to these and other services, the P.O.W.E.R. team may also escort you to your medical appointments as well as provide HIV testing and counseling for your significant other or members of your family, friends, and community.
